Submit an Application

Our applications are now electronic. You can complete and submit your application electronically; no printing needed! We'll receive your data electronically minutes after submission. The application will remain in our queue until you visit our office to finish processing.

Please note:

• The application will show you the price, but you are not able to pay online.
• Most services require an in-person visit to issue the document, after you submit your application. 
• Fictitious Business Name Statements can be printed and mailed with a check, instead of being processed through an office visit.
