ALERT FM Request Form

Thanks to a partnership with Pacific Gas and Electric (PG&E) and ALERT FM, a limited number of ALERT FM devices are available at no cost to those members of the public who qualify to receive a device at no cost. ALERT FM is a device that receives mass notifications via FM radio signals in times of crisis or emergency.

It is important that everyone who can benefit from an ALERT FM device has access to one. This opportunity is intended to provide these devices to those who meet at least one of the following criteria:

• Age 60 years of age or older

• Those with a Disabling Medical Condition

• Those who live in areas at risk for Public Safety Power Shut Offs

• Those with no or limited internet and cell phone service

• And other circumstances

After submitting the Request Form, you will be contacted by staff to arrange a time and location for device pick up.

Those who qualify will sign the  attestation to that effect and a hold harmless agreement. Please print off and return this attestation when you come for device pick up.

Those who feel they can more easily afford to purchase a device on their own are encouraged to do so to allow access for all.
