Medical Marijuana Identification Card Program

It is an optional program for people whose doctors have recommended marijuana for serious health problems.

To process your application or renewal, you need to provide the following:

• A completed Application/Renewal Form (DHS 9042)
• A doctor’s recommendation or a Medical Records Form (CDPH 9044) filled out by your doctor or their staff.
• A government-issued photo ID (like a driver’s license, photo ID card, or military ID). If you're under 18 and don’t have a photo ID, you can use a certified birth certificate. If you choose a primary caregiver, they must also show a photo ID.
• Proof of emancipation if you're an emancipated minor.
• Proof of legal authority if you can’t make your own medical decisions and someone else is representing you.

You must also show proof that you live in Shasta County:

• Current rent or mortgage receipt, or a utility bill with your name on it.
• Current DMV registration with your name on it.
• A California Driver’s License or ID card. If your address is not up to date, you can also provide a DMV Change of Address Certification Card (DL 43).

The address provided must be a physical address. P.O. boxes are not accepted. 

If you’re under 18 and don’t have proof of residency, your parent or legal guardian must show proof on your behalf.

All processing fees are non-refundable.

• $100 for each MMIC requested.
• $50 for each MMIC requested if you are on Medi-Cal at the time of application acceptance. You must bring a copy of your Medi-Cal/Covered California card.
• $0 if you are a CMSP client. You must bring a copy of your CMSP card.
