Why Transition?

Youth and Young Adults with chronic health conditions and disabilities need to prepare for their healthcare needs as they reach adulthood It’s never too early to start planning for this transition.

Important Questions:

  • When will my pediatric doctor stop seeing me?
  • Who will my adult doctor(s) be?
  • Do I need a guardian or conservator?
  • What medication, equipment, or supplies will I need?
  • What health insurance and/or income programs will I be able to get?
  • What are my future plans?
    • Job or career?
    • College?
    • Living on my own?
  • What activities or groups are available in my community to participate in?
  • How can I best advocate for myself and my health care needs?

These are just a few of the questions to ask. Read the more detailed checklist. It’s important to speak with doctors, family, and other support centers in the area (such as Far Northern, GHPP, Rowell Family, CCS case worker, and counselors) to help your transition. CCS coverage ends at midnight on your 21st birthday so it’s VERY important that you and your caregivers know what your adult health plans are before then.
