Medical Marijuana State Identification Card Program

This voluntary program is available to people whose physicians have recommended the use of marijuana for one or more serious medical conditions.

Hours: Must be seen by appointment only: Call Monday - Friday, 8:00 am - 5:00 pm for an appointment.

Eligibility: Must be recommended by a valid physician; patient can designate a primary caregiver and apply for a caregiver identification card.

Application Process: Applicants must apply within their county of residence, have a valid physician's recommendation and complete the Medical Marijuana Application Directions & Application form. All applicants (patient and primary caregiver) must be present during the application process. All applicants must provide a government-issued photo identification (such as a driver's license) and show proof of residency.

Fees: Non Medi-Cal or Medicare: $100 for patient and primary caregiver. Medi-Cal: $50 for patient and primary caregiver. CMSP: $0
