COVID Vaccine Accompaniment Claim

IHSS recipients ages 16 and older who need accompaniment assistance from their provider to obtain a COVID-19 vaccination can submit the COVID Vaccine Accompaniment Claim Form mailed to you by CDSS. This will allow the county to process a payment for an eligible provider for two hours per appointment and a maximum of four hours total. The claim can go back to January 1, 2021..

Note: If case time is already at the maximum statutory limit of 195 or 283 hours per month, the recipient would not qualify for this benefit. For a replacement claim form, please click here. For questions please call 225-5507. Please mail the claim form to:

What is In-Home Support?

In-home supportive services are available for people who are aged, blind or disabled, can not perform activities of daily living and cannot remain safely in their own homes without help. We work with local caregivers to ensure that these people are able to remain in familiar surroundings – safely, comfortably and with as much independence as possible.

Types of Support

Recipients may receive assistance with daily tasks, such as:

  • Bathing
  • Dressing
  • Cooking
  • Cleaning
  • Grooming
  • Feeding

Eligibility

To find out if you are eligible for services, call 530-225-5507, download an Application for In-Home Supportive Services or email IHSS@shastacounty.gov. To be eligible, an applicant must:

  • Be a California resident and reside in his/her own home
  • Be age 65 and over, blind or disabled
  • Be receiving Medi-Cal benefits
  • Have low income as set by SSI/SSP standards (if a person's income exceeds SSI/SSP standards, there may be a share of cost)

Anyone who recognizes that a person is in need of in-home assistance may refer someone to our program. A social worker will conduct an assessment to determine the applicant’s need based on medical condition, living arrangement and resources available. 

If you aren’t able to be a full-time caregiver, you can consider being a back-up provider.

Finding a Caregiver

If an applicant is eligible for services, we can assist you with locating a caregiver through our registry of caregiver providers. A friend or relative may also serve as your caregiver. Once you have selected a caregiver, you will act as your caregiver's employer by supervising tasks and signing time sheets. We will arrange for your caregiver's payment.
